Download & complete the form. Then print the form, get an authorized person's signature and bring the completed form to St. Timothy's office (paper copies of the form are also available at the office if you don't want to deal with technology). It is the responsibility of the person submitting the check request to make sure that receipts or invoices for all expenses being claimed are attached to the check request form before submitting it for payment.

Download & save the completed form to your computer and email the saved file to an authorized person for a signature. Signature person will then print the form and sign and then bring the completed form to the church office. Signature person can also scan and then email the form or take a picture with a phone and email the form to [email protected].
